2022 Occupation Tax Certificate Renewal

2021 Occupation Tax Certificates expire on 12/31/2021.  
10% penalty is assessed on April 1, 2021 and
1.5% interest begins to accrue monthly starting April 1, 2021.

To complete your renewal, please follow the directions on the 2022 Renewal Cover Letter that was mailed with your renewal form, and can be accessed by clicking the link below:

The Renewal & Financial Affidavit are combined onto one form, which was mailed on October 19.  Please ensure your business's email address is up-to-date; this is extremely important as we will begin email renewals next year.  If you have any questions about completing this form, contact Jennifer Prater.

 Here are other helpful forms that you may need, including the E-Verify Affidavit and SAVE affidavit. 

E-Verify Affidavit (this was also included with your renewal form, on the back side of the Renewal Cover Sheet)
If you have previously submitted an E-Verify affidavit of compliance, with E-Verify Number and date of authorization, your E-Verify number should appear on your renewal form; please review.  If this number has not changed, no action is required; if this number has changed, a new affidavit of compliance and affidavit explaining why it changed must be submitted.

 If you are claiming exemption, you must submit this affidavit.

SAVE Affidavit
If you (the applicant signing the renewal) have previously submitted a SAVE affidavit indicating you are a U.S. Citizen, along with a secure and verifiable document, you need not submit this information again.

If you are not a US Citizen, you must submit this form, along with a secure and verifiable document and acceptable immigration document.

Home Office Compliance
If your business is located in your home, you must read, sign and return this form with your renewal.
